Rehabilitation Support:
Deliver high-quality occupational therapy interventions for patients requiring complex medical rehabilitation.
Discharge Planning:
Lead effective discharge planning to ensure smooth transitions from hospital to home or community settings.
Case Management:
Manage your own caseload independently, prioritising patient needs and adapting to changing clinical demands.
Multidisciplinary Working:
Collaborate with medical and allied health professionals across various wards to provide holistic care.
Treatment Planning:
Adapt and review therapy plans to ensure they are responsive to patient progress and evolving needs.
Qualifications / Requirements
HCPC registered Occupational Therapist. Experience within acute or medical rehabilitation settings. Strong discharge planning and case management skills.
