At Aveanna, our services include Skilled Nursing, Physical Therapy, Occupational Therapy, and Home Health Aide support delivered in patients' homes. We serve diverse populations across all ages, including patients with chronic conditions, behavioral health needs, skilled nursing requirements, and therapy needs. Employees who work at least 130 hours per calendar month over this Initial Measurement Period will be invited to enroll in our ACA Benefits Plans, which offer an array of enhanced benefit plan options. If you meet the requirement under the twelve-month Measurement Period, you will also be eligible for your choice of major medical plans, dental and vision coverage. Benefit eligibility can vary and is dependent upon employment status and employment location. What You'll Do Follow policies and procedures and conforms to all applicable patient care standards, licensure regulations and state laws Provide occupational therapy services as prescribed by the physician Provide a written evaluation defining occupational therapy needs, goals and expected outcomes in conjunction with referring physician's Plan of Treatment Provide a written Plan of Care from the nursing staff to provide continuing care Carry out appropriate techniques according to identified goals and treatment programs Participate in coordination of services (including discharge planning), with health care team, physicians, parent/guardians, nurses, and appropriate others Administer care to ensure safety for the patient Update self on the patient's clinical condition Maintain effective communication between staff, health care team members, and family. Serves as a resource consultant for clinical staff Provide physicians and Aveanna with written evaluation of patient's needs, goals, and progress on a weekly basis or as ordered Attends patient case conferences when requested. Attends and participates in in-services as indicated. Supervises other personnel (COTA, HHA) when required Qualifications A certification in Occupational Therapy in the state of application Current and unrestricted license in the state of practice Current CPR certification (with hands-on component) or able to obtain one Current TB test or able to obtain one Must have a drivers license and reliable transportation Aveanna Healthcare is an Equal Opportunity Employer and encourages applicants from diverse backgrounds to apply.
