Job Description

Renewal Rehab is seeking a Licensed Occupational Therapist to provide high-quality, patient-centered care in skilled nursing and post-acute settings. In this role, you will evaluate functional abilities, develop personalized treatment plans, and deliver evidence-based interventions that help primarily senior patients maximize independence and quality of life. You'll collaborate closely with an interdisciplinary team, document progress in EMR, educate patients and caregivers, and contribute to outcome-focused rehab programs, all within a supportive, growth-focused culture with mentorship and advancement opportunities. Responsibilities Evaluate patients' functional status, cognitive abilities, and ADL performance using standardized assessments. Develop and implement individualized, evidence-based occupational therapy treatment plans for primarily senior patients. Provide therapeutic interventions to improve strength, coordination, balance, self-care, and safety in daily activities. Document evaluations, treatments, goals, and patient progress accurately and timely in EMR systems. Collaborate with an interdisciplinary team, including PTs, SLPs, nursing, and physicians, to coordinate care. Educate patients, families, and caregivers on home programs, adaptive strategies, and safety techniques. Participate in care conferences and discharge planning to support safe transitions and continuity of care. Adhere to regulatory, ethical, and documentation standards while upholding Renewal Rehab's clinical quality expectations. Contribute to outcome-focused rehab programs and quality improvement initiatives within partner facilities. Engage in ongoing professional development, mentorship, and continuing education opportunities.
