In this vital role, you will empower older adults to regain independence, improve their quality of life, and achieve their personal goals through tailored therapeutic interventions. Your expertise will help clients navigate the challenges of aging, disabilities, and recovery, making a meaningful difference every day. This position offers flexible hours and the opportunity to work with a diverse population in a vibrant outpatient setting. Responsibilities Assess clients' functional abilities and develop personalized care plans focused on improving activities of daily living (ADLs) and instrumental activities of daily living (IADLs) Implement evidence-based therapeutic interventions to enhance mobility, strength, coordination, and cognitive functioning in geriatric patients Assist patients with discharge planning by coordinating with families, caregivers, and healthcare teams to ensure smooth transitions from therapy to home or other care settings Conduct comprehensive evaluations for individuals with disabilities or age-related conditions, including those with autism or developmental delays when applicable Document patient progress accurately and maintain detailed records aligned with medical terminology, physiology knowledge, and anatomy understanding Collaborate with multidisciplinary teams including speech therapists, physicians, nurses, and social workers to optimize patient outcomes Provide education to patients and families on safety strategies, adaptive equipment use, and activity modifications to promote independence Requirements Valid license as an Occupational Therapist (OTR/L) with current certification to practice in your state Proven experience working in outpatient clinics with geriatric populations; experience in skilled nursing facilities, ILF/ALF or post-acute care is highly desirable Strong knowledge of medical terminology, physiology, anatomy, and therapeutic techniques relevant to geriatrics and disabilities Experience assisting with ADLs, discharge planning, case management, and working with individuals with autism or developmental delays preferred Ability to work independently while collaborating effectively within an interdisciplinary team Excellent communication skills for patient education and documentation purposes Join us in delivering compassionate care that transforms lives!
