$80,000-$150,000 annually (Per-Visit Model)

What You'll Be DoingRestore Function And Confidence Perform comprehensive in-home occupational therapy evaluations Develop individualized treatment plans focused on ADLs and independence Deliver hands-on interventions to improve strength, coordination, and motor skills Conduct home safety assessments and recommend adaptive equipment Help patients achieve their highest level of functional performance Collaborate Across The Continuum Work closely with Physical Therapists, nurses, physicians, and interdisciplinary clinicians Educate patients and caregivers on safety and compensatory strategies Document visits accurately and efficiently Support exceptional patient outcomes and quality of life Required Qualifications Active Minnesota Occupational Therapist license Valid driver's license and reliable transportation Strong clinical reasoning and communication skills Ability to work independently and manage your schedule effectively Preferred Qualifications Home health experience Experience with medically complex or geriatric populations Familiarity with EMR systems Passion for patient-centered, relationship-driven care Independence Is More Than A Goal.
