What You'll Do Perform occupational therapy evaluations in patients' homes. Develop individualized treatment plans that improve safety, independence, and quality of life. Provide interventions to enhance activities of daily living (ADLs), mobility, home safety, and fall prevention. Recommend adaptive equipment and home modifications. Collaborate with physicians, nurses, caregivers, and the interdisciplinary care team. Complete timely, accurate documentation in accordance with home health standards. Qualifications Current Occupational Therapist license in the state of practice. Graduate of an accredited Occupational Therapy program. Valid driver's license and reliable transportation.
