Job Summary:
The role of the Occupational Therapist at Active Therapy is to assist patients in Assisted Living settings achieve their highest level of independence no matter where they are at in their journey. This is achieved through working closely with patients, their families, and the Assisted Living Community's staff to develop individualized treatment plans, plans of care and maintenance programs to aide in the highest level of functionality.
Essential Functions:
Evaluates the patient's needs for cognitive support, along with activities of daily assistance so that their highest level of independence no matter where they are at in their journey is achieved Assess patient's home environment to make recommendations on use and/or if adjustments are needed Establishes and administers a treatment program with specific goals determined according to the patient's capacity Works with patients to increase strength and endurance Creates, communicates and train on post occupational therapy maintenance programs Assists patients, families and assisted living community staff obtain equipment to support the patient's individualized needs Provides education to the patient, family/caregiver and assisted living community staff on information regarding the patient's specific care needs/individualized adjustments to plan of care Maintains professional knowledge and awareness of best practices to provide patients with the best individualized treatment program available Documents patient's evaluations, treatments, progress notes, communications and plans associated with their care/services Provides ongoing communication to patients, families and assisted living community staff Complies with federal and state regulations, along with follows organizational policies Other duties as assigned
Certifications, Licenses, Registrations and Other Requirements:
Graduated from an accredited Occupational Therapist program Valid Occupational Therapy License in the State of Minnesota Certified in Basic Life Support Must pass all applicable background checks and health screens as required by the
Minnesota Department of Health Physical Demands:
Must be able to travel to provide support to multiple communities in Minnesota Must be able to provide hands-on care as needed to patients. This may include lifting, bending, pushing, and twisting to assist patients.

$40.00•$46.00 per hour
