What You'll Be Doing As an Occupational Therapist, you'll practice at the top of your license while helping patients remain safe, functional, and independent in their own homes.
Responsibilities include:
Providing in-home occupational therapy evaluations and treatment services. Assessing patient functionality, safety, and independence within the home environment. Developing individualized care plans with measurable goals. Improving activities of daily living, fine motor skills, strength, and overall function. Recommending adaptive equipment and home modifications that support independence. Completing home safety assessments to reduce fall risks and promote confidence. Collaborating closely with nurses, physical therapists, speech therapists, physicians, and caregivers. Maintaining accurate and timely EMR documentation. Qualifications Current Minnesota Occupational Therapist license. Valid driver's license and reliable transportation. Ability to work independently in a field-based setting. EMR experience preferred.
