Home Health Care, Inc. is seeking an experienced OTR/L to complete Start of Care visits throughout the Twin Cities Metro and surrounding communities , with premium per-visit compensation and the opportunity to earn even more when you choose to pick up weekend admissions. Want to add a few admissions and increase your paycheck? The opportunity is there. You Set the Clinical Direction From the First Visit A strong Start of Care isn't simply paperwork. It's the foundation for the patient's entire episode of home health. You'll determine functional needs, assess the home environment, identify risks, understand caregiver support, establish goals, and create the initial roadmap that helps guide the interdisciplinary team. For an experienced OT, this is an opportunity to use far more than treatment skills. You'll use your assessment abilities, clinical judgment, communication skills, Medicare knowledge, and ability to quickly understand complex situations. What You'll Be Doing Complete comprehensive Start of Care Occupational Therapy evaluations in patient homes. Perform detailed assessments of ADLs, functional mobility, cognition, upper-extremity function, caregiver support, and home safety. Complete OASIS and required admission documentation accurately and within applicable timeframes. Develop individualized plans of care based on clinical findings, physician orders, and patient goals. Identify fall risks, environmental concerns, and barriers to independence. Recommend adaptive equipment and appropriate home modifications. Educate patients and caregivers on safety, energy conservation, and adaptive techniques. Collaborate with nursing, PT, SLP, social work, physicians, and other members of the care team. Complete timely electronic documentation and maintain regulatory compliance. Travel throughout the Twin Cities Metro and surrounding communities based on referral needs. This Role Is Built for an Experienced Home Health Clinician We're especially interested in Occupational Therapists who already understand how different an admission visit is from a routine treatment visit. You should be comfortable walking into an unfamiliar situation, asking the right questions, identifying priorities quickly, completing comprehensive documentation, and making confident clinical decisions independently. Required Qualifications Current Minnesota Occupational Therapist (OTR/L) license Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong assessment, documentation, communication, and organizational skills Confidence working independently in patient homes Preferred Qualifications Previous home health experience OASIS experience Start of Care / admission experience Knowledge of Medicare home health regulations Experience with medically complex, orthopedic, neurological, or geriatric populations Make Admissions Your Specialty and Your Earning Opportunity If you're an experienced home health OT who enjoys the challenge of meeting a patient for the first time and figuring out exactly what they need, we'd like to put that expertise to work.
