The First Visit Can Shape Everything That Happens After It. Before the nurse makes the next visit. Before therapy establishes its rhythm. Before the aide begins care. Before the interdisciplinary team has weeks of notes and history to work from. Someone has to establish the starting point. Home Health Care, Inc. is hiring an experienced Occupational Therapist (OTR/L) to complete Start of Care admissions throughout the Twin Cities Metro and surrounding communities. This is a role for an OT who understands that a great admission isn't simply about completing an assessment. It's about setting the patient and the entire home health team up for what comes next. Set the Direction From Day One During a Start of Care visit, you're helping establish the foundation for the patient's episode of care. You'll assess the patient in their home, identify functional and safety concerns, understand caregiver capabilities, complete OASIS documentation, establish appropriate goals and plans of care, and help determine which services and interventions need to happen next. The quality of that first assessment matters. When it's thorough, the clinicians who follow you have a clearer picture of the patient. When risks are identified early, the team can respond sooner. When the plan of care reflects what's actually happening in the home, everyone starts from a stronger position. Why This Role Is Different $100 per Start of Care visit Monday-Friday $120 per Start of Care visit on weekends Optional weekend admissions at the higher rate Mileage reimbursement for eligible travel Full-Time opportunity Start of Care-focused position Comprehensive assessment and OASIS responsibilities MedBridge continuing education access Cell phone reimbursement Licensure reimbursement Annual scrub reimbursement Comprehensive onboarding and clinical support Established Medicare-certified home health organization Your Work Becomes the Starting Point for the Team A referral can tell you diagnoses, medications, recent hospitalizations, and physician orders. Your admission helps turn that information into a working clinical plan . You'll evaluate how the patient is actually functioning at home. Can they complete essential ADLs safely? What does their mobility look like? Is cognition affecting their ability to follow instructions or manage routines? Is the caregiver capable of providing the assistance everyone assumed they could? Are there fall risks or environmental barriers? Does the patient need additional disciplines involved? Your assessment helps answer the questions the rest of the team needs answered. What You'll Be Doing Complete comprehensive Start of Care Occupational Therapy evaluations Perform and document required OASIS assessments Assess ADLs, cognition, functional mobility, upper-extremity function, caregiver support, and home safety Establish the patient's functional baseline Identify immediate clinical, safety, and environmental concerns Develop individualized plans of care based on assessment findings, physician orders, and patient goals Recommend adaptive equipment and home modifications when appropriate Educate patients and caregivers on safety and adaptive strategies Coordinate with nursing, PT, SLP, social work, physicians, and other clinicians Ensure admission documentation is accurate, complete, and timely Travel throughout the Twin Cities Metro and surrounding service area based on referral needs This Role Needs an Experienced Clinician Start of Care isn't the ideal place to learn basic home health assessment on the fly. We're looking for an OT who can enter a new patient environment, conduct a comprehensive assessment, organize a large amount of clinical information, complete accurate documentation, and confidently establish what should happen next. What You Bring Current Minnesota Occupational Therapist (OTR/L) license Current CPR certification Valid driver's license and reliable transportation Ability to travel throughout an approximately 75-mile service radius Strong assessment and clinical reasoning skills Excellent organization and documentation Ability to communicate effectively with patients, caregivers, physicians, and interdisciplinary clinicians Confidence working independently in patient homes Especially Valuable Experience Previous home health experience OASIS experience Start of Care or admission experience Knowledge of Medicare home health requirements Experience with medically complex, orthopedic, neurological, or geriatric populations Want Another Earning Lever? Weekend Admissions Are Optional. Your regular role is weekday-focused. If you'd like additional earning opportunities, optional weekend Start of Care visits pay $120 per visit , compared with $100 per weekday Start of Care visit . That gives you the opportunity to pick up additional admissions when they fit your availability without making regular weekend work a requirement. Some OT Roles Continue the Plan. This One Helps Set It. There's a different kind of responsibility that comes with being one of the first clinicians through the door. You're establishing the baseline. Identifying the priorities. Documenting what the team needs to know. And helping create a plan that other clinicians will build upon throughout the patient's episode of care. If you enjoy comprehensive assessment, OASIS, admissions, clinical decision-making, and being at the front end of patient care , this may be the kind of OT role you've been looking for. Earn $100 per weekday Start of Care visit plus mileage reimbursement , with opportunities to earn $120 per visit for optional weekend admissions . Want to stand out from other applicants?
