What You'll Do As an Occupational Therapist, you'll help patients maximize independence and improve quality of life while remaining safe in their own homes. Responsibilities include providing evaluations and treatment services, assessing functionality and home safety, developing individualized care plans, recommending adaptive equipment, and collaborating closely with physicians, nurses, and fellow therapists to ensure exceptional outcomes. You'll also educate patients and families while helping individuals regain confidence and maintain the daily activities that matter most to them. Responsibilities Provide in-home occupational therapy evaluations and treatment services. Assess patient functionality, safety, and independence within the home. Develop individualized care plans with measurable goals. Implement therapeutic interventions to improve daily living skills. Recommend adaptive equipment and home modifications. Collaborate with nurses, therapists, and physicians to coordinate care. Complete documentation accurately and timely within the EMR. Qualifications Current Minnesota Occupational Therapist license. Valid driver's license and reliable transportation. Ability to work independently in a field-based environment. Strong communication and patient education skills. EMR experience preferred.
