Responsibilities:

Design and implement individualized occupational therapy plans to address the unique needs of our residents, including those with cognitive and physical impairments Conduct thorough assessments and evaluations to identify areas of strength and weakness, and develop targeted interventions to promote functional ability and independence Develop and lead group and individual therapy sessions, using a variety of evidence-based techniques and strategies Collaborate with our interdisciplinary team to ensure seamless care coordination and communication Maintain accurate and up-to-date records of patient progress and treatment plans Participate in quality improvement initiatives and contribute to the development of clinical policies and procedures
