Licenses:
OT-MT(Copy needed)
Certifications:
OTR-National Board for Certification in Occupational Therapy(Copy needed); BLS-AHA(Copy needed)
AND Occupational Therapist, Outpatient SSN Required DOB Required Work History:
Should not be more than 90 days of gap in last 1 year
