About Facility:

Home Health Radius rule: Traveler's permanent address must be 50 miles outside of home unit

Department Information:

o 6 visits a day o Minimum Years of Experience required? 1 year o Will you accept a first-time traveler? Yes, if seasoned to home health

Patient Types/Common Diagnoses:

o Chronic diseases, wound care, IV, pleurx, wound vac Required License, National Certification, Certs (BLS, ACLS, etc.), or other position specific requirements such as Driver's License and/or

Insurance:

o Driver's License and proof of Insurance required o

OT:

BLS; State Licensure required Skills required: o IV, pleurx, wound vac, wound care, assessment, disease teaching, OASIS Support within the

Department:

o Departmental leadership
