Job Description
Heath Village is a constantly growing and unique retirement community providing multiple levels of housing and healthcare for close to 400 residents and is located on a 100+ acre campus. Apply today to learn more about our vibrant community! The Meadows Nursing and Rehabilitation Facility, part of Heath Village Retirement Community, is a New Jersey state-licensed, 108 bed facility. Certified and rated five stars by Medicare, our health care center offers comprehensive and professional nursing services for both temporary and extended stays. Extensive rehabilitation services are available on-site. All residents, as well as members of the community-at-large can avail themselves of our premier rehabilitation and nursing programs. The Meadows offers four long-term households and two sub-acute/short-term ones. Our rehab team is seeking full time Occupational Therapist to assist with current and upcoming caseload needs. Provides Occupational Therapy services to patients/residents according to the physicians direction, and initiates referrals when appropriate to applicable team members Monitors patient response to treatment/intervention and customizes plan in order to attain desired outcomes in accordance with the orders of attending physician. Create appropriate skilled rehabilitative treatment plan and goals based upon completed evaluation of resident/patient. Accurately document results of patient treatments, response to therapeutic intervention, and discontinuation of services, into computer system in a timely fashion. Performs and records accurate and timely billing information to current computer system. Communicates OT rehabilitation program plans to resident, family, and other team members via team meetings, individual conferences, utilization review, and other means of communication.
Requirements:
Bachelors of Science, Occupational Therapy Must be licensed in NJ.Hours:
