In this skilled nursing rehabilitation role, you'll identify what stands between residents and their everyday activities, then provide treatment and practical strategies that help them participate more independently. What You'll Do Assess self-care performance, functional transfers, upper-extremity abilities, cognition, and activity tolerance. Develop individualized plans of care based on evaluation findings, resident priorities, and anticipated discharge needs. Use purposeful activities to improve daily living skills and address limitations affecting task completion. Recommend adaptive equipment and educate residents and caregivers on safe techniques and compensatory strategies. Monitor functional gains, revise treatment goals, and supervise COTA services. Maintain thorough clinical records and communicate recommendations to nursing and the interdisciplinary care team. What We Offer $50

• 55 per hour.
