A fast-growing, Certified Home Health Agency (CHHA) is seeking a Full-Time Occupational Therapist (OT) to provide home care visits to patients throughout Queens. Develop individualized, patient-specific plans of care. Provide skilled Occupational Therapy services in the patient's home. Complete clinical documentation accurately and submit it to the agency in a timely manner. Monitor patient progress and modify the plan of care as appropriate. Promote patient safety, independence, and satisfaction. Communicate effectively with patients, families, and the home care team. Qualifications Current NYS Occupational Therapist license required Home health/home care experience preferred Experience with an electronic medical record (EMR) preferred, but not required Strong clinical, communication, and organizational skills Ability to work independently and manage a field-based schedule If you are an Occupational Therapist looking for a stable Monday-Friday position with excellent compensation, benefits, and the flexibility of working in the field, we would love to hear from you!
