2992010676-2 Apply for this job now Job Description FOX Rehabilitation seeks a licensed Occupational Therapist to provide evidence-based, geriatric rehab services in patients' homes and senior living communities. In this autonomous, mobile role, you will evaluate functional abilities, develop and deliver individualized treatment plans, document per regulatory and payer standards, and collaborate closely with an interdisciplinary team. Responsibilities Provide in-home and senior living occupational therapy evaluations and treatments for older adults Develop, implement, and adjust individualized, evidence-based treatment plans Document assessments, goals, and progress in accordance with regulatory and payer requirements Educate patients, families, and caregivers on home programs, safety, and adaptive strategies Collaborate with an interdisciplinary care team to optimize functional outcomes Utilize productivity tools and clinical resources to manage a mobile, community-based caseload Required Skills Occupational therapy evaluation and treatment Geriatric rehabilitation Functional mobility and ADL training Home safety and fall risk assessment Therapy documentation and EMR use Care planning and goal setting Patient and caregiver education Interdisciplinary team collaboration Knowledge of Medicare and payer regulations Use of adaptive equipment and assistive devices
