Job Description

Occupational Therapist Job Summary:
The Grand Rehabilitation and Nursing at River Valley, a prestigious healthcare provider, is seeking a compassionate and skilled Occupational Therapist (OT) to join our team in Poughkeepsie, New York. As a valued member of our rehabilitation team, you will play a vital role in helping our residents achieve their wellness goals and regain their independence.
Responsibilities:
Evaluate and develop treatment plans for residents based on medical diagnosis Implement treatment plans to improve residents' functional independence Assess and document progress of residents in detailed re-certifications, progress reports, and daily notes Maintain documentation of Resident care services by auditing Resident & Department records Supervise occupational therapy assistants and participate in care plan meetings and documentation Plan, develop, and organize OT-related reports and data tracking as directed by the Rehab Director Assist in departmental projects as needed
Requirements:
Graduate of an accredited Occupational Therapy Program Current New York State OT License Experience is a plus
