Days Northgate Health Care Facility is looking to add an Occupational Therapist (OT) to its rehabilitation team. In this role, you'll evaluate residents, develop individualized treatment plans, and provide therapy services that help residents regain independence, improve daily living skills, and achieve their highest level of function. RESPONSIBILITIES
Evaluate residents and develop individualized occupational therapy treatment plans Provide skilled therapy services focused on improving functional abilities, ADLs, mobility, and quality of life Establish short- and long-term rehabilitation goals and monitor resident progress Collaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations Complete all required documentation, assessments, care plans, and billing records accurately and timely Provide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs Recommend and coordinate adaptive equipment and durable medical equipment (DME) needs Participate in care conferences, discharge planning, staff education, and quality improvement initiatives Perform additional duties as assigned
REQUIREMENTS
Master's Degree in Occupational Therapy from an accredited program approved by the American Occupational Therapy Association Valid New York State Occupational Therapist License required Newly licensed Occupational Therapists encouraged to apply Skilled nursing facility experience preferred Strong clinical, documentation, communication, and problem-solving skills Ability to work collaboratively within an interdisciplinary healthcare team Commitment to resident-centered care and rehabilitation excellence
