Description Occupational Therapist (OT) - Post-Acute Rehab (SNF) - PRN Why Concept Rehab Privately owned and operated by therapists for 40+ years Supportive, clinician-focused culture Flexible PRN opportunities that fit your schedule Ongoing education, resources, and career growth Make a meaningful impact on patients, families, and facility partners Role Summary As a PRN Occupational Therapist, you will evaluate and treat patients with challenges related to mobility, cognition, strength, and activities of daily living. You'll collaborate with interdisciplinary teams to deliver individualized, patient-centered care in accordance with AOTA standards. Requirements Key Responsibilities Evaluate patients within 24-48 hours of physician orders Develop and implement individualized treatment plans Supervise OTAs and support care delivery Communicate with physicians, care teams, patients, and families Document care, progress, and billing accurately Participate in care planning and discharge processes Provide caregiver and staff training Recommend adaptive equipment to support patient outcomes What You Bring Strong communication and interpersonal skills Clinical judgment and problem-solving ability High level of professionalism, ethics, and confidentiality Passion for delivering compassionate, patient-centered care Qualifications Bachelor's, Master's, or Doctorate in Occupational Therapy Active Occupational Therapist license (state-specific)
