Community Skilled Health Care Centre, 1320 Mahoning Ave NW, Warren, OH 44483 Help Residents Regain Independence - Join Our OT Team! Community Skilled Health Care Centre is seeking a skilled and compassionate Occupational Therapist (OT) to join our therapy department. We offer both full-time and part-time opportunities for OTs who are passionate about making a real difference in the lives of our short-term rehab and long-term care residents.
Your Role:
Evaluate residents' physical and cognitive abilities and develop personalized treatment plans Deliver evidence-based occupational therapy interventions Focus on improving residents' ability to perform activities of daily living (ADLs) Collaborate closely with nursing, rehab, and other care team members Maintain accurate and timely clinical documentation
What You Need:
Valid Ohio Occupational Therapist license Experience in a skilled nursing facility (SNF) or rehab setting preferred Ability to work well independently and as part of a team Dedication to high-quality, resident-centered care
