Responsibilities:

Assess patients' cognitive, physical, and sensory skills to develop effective therapy plans Implement and modify therapy plans to address patients' specific needs and goals Collaborate with interdisciplinary teams to ensure comprehensive care and achieve optimal outcomes Monitor and document patients' progress, adjusting therapy plans as needed Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination Participate in quality improvement initiatives to enhance patient satisfaction and outcomes
