30 Points Per Week Position Overview Caretenders VNA is seeking an experienced Travel Occupational Therapist (OT) to join our Home Health team. This position offers the opportunity to provide individualized, patient-centered care in the home setting while helping patients improve their functional independence, safety, and quality of life. The ideal candidate will have prior home health experience, strong clinical assessment skills, and proficiency with Homecare Homebase (HCHB) documentation. Coverage Area The Occupational Therapist will provide care throughout: Fairfield County, OH Perry County, OH Hocking County, OH Schedule Monday•Friday 8:00 AM•4:30
PM On-Call/Weekend Requirement:
None Mileage Reimbursement Yes•Mileage is reimbursed for travel between patient visits, from the first patient visit to the last patient visit of the day. Key Responsibilities Perform comprehensive occupational therapy evaluations in the home setting. Complete Start of Care (SOC) assessments, recertifications, and routine therapy reassessments. Develop and implement individualized treatment plans designed to improve patient function, mobility, safety, and independence with daily living activities. Provide skilled occupational therapy treatment interventions according to physician orders and agency guidelines. Educate patients, families, and caregivers on adaptive equipment, home safety, energy conservation, and therapeutic exercises. Monitor patient progress and modify treatment plans as appropriate to achieve optimal outcomes. Collaborate with physicians, nurses, therapy staff, and other interdisciplinary team members to coordinate care. Maintain accurate, timely, and compliant documentation within the electronic medical record (EMR) system, including Homecare Homebase (HCHB). Required Qualifications Current Occupational Therapist (OT) license. Minimum of one year of recent Home Health experience. Experience using Homecare Homebase (HCHB) documentation. Strong assessment, documentation, and patient education skills. Ability to work independently and effectively manage a home-based patient caseload. Preferred Qualifications Candidates will stand out with: Experience in Lymphedema Therapy. Additional knowledge of specialty rehabilitation techniques. Excellent communication and relationship-building skills. Strong organizational and time-management abilities. Experience working in a multidisciplinary home health environment.
