The hiring rate is dependent upon several factors, including but not limited to education, training, work experience, terms of any applicable collective bargaining agreement, seniority, etc. You will have the opportunity to work in the patient's own environment engaging family and caregivers. Our multi-disciplinary Home Health team consists of Physical Therapists, Occupational Therapists, Speech Language Pathologists, RN's, Medical Social Workers, Registered Dietitians and Bath Aides. Our dynamic team meets regularly providing the opportunity to collaborate with supportive peers to coordinate care and achieve optimal outcomes. Enjoy a lifestyle inspired by the great outdoors! The town of Florence is located on the Oregon Coast at the mouth of the Siuslaw River and is known for its breathtaking beaches, rolling sand dunes, numerous lakes with hiking trails and a picturesque historic downtown Job Summary Responsible for the evaluation and treatment of patients within the scope of Occupational Therapy, determining plan of care and setting goals of treatment. May provide oversight to therapy assistants, therapy aides and/or students in following the plan-of-care and treatment of patients. Works collaboratively as a part of a patient care team toward the best outcomes for patients.
What you will do:
Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation and team members. Completes patient treatment according to plan of care and modifies plan to meet patient needs, and updates plan as appropriate. Completes documentation accurately, timely, according to regulatory and state licensure requirements, and within professional standards. Collaborates with other healthcare professionals as necessary to ensure smooth continuous care of the patients. May act as a liaison with nursing, medical and other therapy staffs to facilitate problem solving and coordination of other services and act as an educational resource. May mentor and provide oversight to new caregivers, occupational therapy assistants, and/or therapy aides. May provide clinical instruction and training to students. May recommend and participate in development of new services and programs, quality improvement and monitoring activities. Maintains appropriate clinical records. May work with OT student affiliation programs. May participate in staff meetings, patient care meetings, educational in-services, caregiver orientation and other professional activities.
What you bring:
Education Bachelor's, Master's or Doctorate Degree Required:
Occupational Therapy accredited by the Accreditation Council for Occupational Therapy Education Experience One year experience as Occupational Therapist preferred. Home Health experience preferred.
