Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Lehigh Valley, Pennsylvania, including Allentown, Bethlehem, Easton, Whitehall, Emmaus, Macungie, Nazareth, Hellertown, Center Valley, and surrounding communities. As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence needed to perform everyday activities while remaining safe and independent in their own homes. What You'll Do Complete occupational therapy evaluations, reassessments, and discharge visits Develop individualized treatment plans based on patient goals and functional needs Provide skilled occupational therapy services in patients' homes Assess home environments and recommend adaptive equipment and safety modifications Educate patients and caregivers on therapeutic techniques, home safety, energy conservation, adaptive equipment, and activities of daily living Monitor patient progress and modify treatment plans as appropriate Collaborate with physicians, nurses, and the interdisciplinary care team Complete timely and accurate clinical documentation using WellSky Qualifications Active Pennsylvania Occupational Therapist License Required Valid Driver's License and Reliable Transportation Strong Clinical Assessment, Communication, and Documentation Skills Home Health Experience Preferred but Not Required OASIS Experience Preferred Experience with WellSky Home Health is a plus Passion for Delivering Exceptional Patient-Centered Care Why Human Touch Home Health?
