Interim HealthCare seeks a compassionate Occupational Therapist to deliver patient-centered home health services. In this role, you will assess functional abilities, develop individualized treatment plans, and provide hands-on therapy to help clients improve safety, independence, and quality of life at home. Responsibilities Perform comprehensive OT evaluations in the home setting Develop and implement individualized treatment plans and goals Provide therapeutic interventions to improve function, mobility, and safety Train patients and caregivers on adaptive techniques and equipment use Collaborate with nurses, therapists, and physicians on care plans Document assessments, progress notes, and discharge summaries accurately and timely Monitor patient progress and adjust care plans as needed Ensure compliance with clinical, regulatory, and agency standards Educate patients and families on home safety and energy conservation Participate in case conferences and quality improvement activities Required Skills Occupational therapy evaluation and treatment planning Knowledge of home health regulations and documentation standards (e.g., OASIS) Therapeutic exercise and functional training Activities of daily living (ADL) and instrumental ADL training Use and fitting of adaptive equipment and assistive devices Patient and caregiver education Home safety assessment and modification recommendations Electronic health record (EHR) documentation Interdisciplinary care coordination and communication Pain management and positioning techniques
