Bethlehem, PA Join a Growing Home Health Team Dedicated to Exceptional Patient Care Patient Outreach Healthcare, Inc. is a Medicare-certified home health agency proudly serving patients throughout Lehigh, Northampton, Berks, Bucks, Montgomery, Carbon, and Philadelphia Counties . We are currently seeking a compassionate, motivated, and skilled Occupational Therapist (PRN) to join our growing clinical team. As a Home Health Occupational Therapist, you will help patients regain independence, improve safety in the home, and maximize their ability to perform daily activities while delivering high-quality, patient-centered care.
Responsibilities:
Perform comprehensive occupational therapy evaluations in patients' homes. Develop individualized treatment plans based on physician orders and patient needs. Provide skilled occupational therapy interventions to improve functional independence. Assess home safety and recommend adaptive equipment and environmental modifications. Educate patients, families, and caregivers on therapeutic techniques and home safety. Collaborate with physicians, nurses, physical therapists, speech therapists, and other members of the interdisciplinary team. Complete timely, accurate, and compliant clinical documentation. Participate in care conferences and communicate changes in patient condition. Qualifications Current Pennsylvania Occupational Therapist (OT) license in good standing. Graduate of an accredited Occupational Therapy program. Current CPR certification. Valid driver's license, reliable transportation, and automobile insurance. Excellent communication, organizational, and documentation skills. Previous home health experience is preferred but not required.
