Home Health Occupational Therapist (OT) -
PRN Coverage Area:
Philadelphia & Delaware Counties (Additional territories available) Position Overview A Medicare-certified, Pennsylvania-licensed home health agency is seeking a PRN Occupational Therapist (OT) to provide skilled therapy services to patients in their homes throughout the Greater Philadelphia region. Patients are typically recovering from surgery, hospitalization, illness, injury, or disability, with a focus on maximizing independence and safety in the home environment. Responsibilities Complete occupational therapy evaluations in patients' homes Develop and implement individualized treatment plans Provide therapy focused on ADLs, functional mobility, strength, coordination, and safety Assess home environments and recommend adaptive equipment or home modifications Educate patients and caregivers on home safety and fall prevention strategies Monitor patient progress and adjust treatment plans as needed Collaborate with RNs, PTs, SLPs, social workers, and other members of the care team Complete accurate and timely EMR documentation Qualifications Active Pennsylvania Occupational Therapist license Home Health experience preferred OASIS experience preferred Strong assessment and patient education skills Ability to work independently in a community-based setting Valid driver's license and reliable transportation Medical, dental, and vision insurance are available to qualified candidates who meet eligibility requirements.
