Job Description

Home Health Occupational Therapist (OT) - PRN /
Hourly Territory:
Philadelphia, Montgomery, Bucks, Delaware & Chester Counties A Medicare-certified and Pennsylvania state-licensed home health agency is seeking a PRN Occupational Therapist (OT) to provide skilled therapy services to patients in their homes throughout the greater Philadelphia region. Patients are typically recovering from surgery, hospitalization, illness, injury, or disability, and therapy focuses on helping them safely regain independence at home. Position Summary The Occupational Therapist provides skilled therapy focused on improving patients' ability to perform activities of daily living (ADLs) and safely function within their home environment. The OT works independently in the field while collaborating with the interdisciplinary care team to support patient recovery and independence. Responsibilities
  • Perform occupational therapy evaluations in patients' homes
  • Develop and implement individualized treatment plans based on physician orders and patient needs
  • Provide therapy focused on ADLs, functional mobility, coordination, strength, and safety
  • Assess the home environment and recommend adaptive equipment or home modifications
  • Educate patients and caregivers on adaptive techniques, fall prevention, and home safety
  • Monitor patient progress and adjust treatment plans as appropriate
  • Collaborate with RNs, PTs, speech therapists, and social workers to coordinate care
  • Maintain timely and accurate clinical documentation in the EMR system Qualifications
  • Active Pennsylvania Occupational Therapist (OT) license
  • Home health experience preferred
  • Strong clinical assessment and patient education skills
  • Ability to work independently in a home-based setting
  • Valid driver's license and reliable transportation
