Position Summary:
The Cognitive Therapist is responsible for assessing, developing, and implementing individualized cognitive rehabilitation programs for patients receiving in-home healthcare services. This role focuses on helping clients improve memory, attention, problem-solving, executive function, and other cognitive skills affected by injury, illness, or developmental conditions. The therapist will work closely with the care team—including nurses, physical therapists, occupational therapists, and caregivers—to ensure a coordinated, patient-centered approach that supports functional independence and quality of life.
Key Responsibilities:
Patient Care & Therapy Delivery Conduct comprehensive cognitive and functional assessments to determine baseline abilities and identify deficits Develop individualized treatment plans tailored to patient needs, goals, and cultural considerations Provide direct therapy in patients' homes or community settings, focusing on improving attention, memory, reasoning, planning, and executive functions Use evidence-based interventions, compensatory strategies, and assistive technologies to support daily living and independence Monitor and document patient progress, modifying treatment plans as needed Care Coordination & Communication Collaborate with nurses, caregivers, and other therapists to integrate cognitive goals into the overall care plan Educate patients, families, and caregivers on techniques to support cognitive function outside of therapy sessions Communicate effectively with managed care organizations (MCOs) and primary care providers regarding patient status and therapy outcomes Compliance & Documentation Maintain accurate and timely clinical documentation in accordance with Pennsylvania state regulations, payer requirements, and agency policies Ensure compliance with HIPAA and patient privacy standards Participate in quality assurance reviews and audits as required Professional Development Stay updated on new research, tools, and techniques in cognitive therapy and neurorehabilitation Attend required agency training and professional continuing education sessions
Qualifications:
Master's degree or higher in Speech-Language Pathology, Occupational Therapy, Neuropsychology, or related field with specialized cognitive rehabilitation training Current Pennsylvania licensure and certifications as applicable (e.g., CCC-SLP, OTR/L) Minimum 1 year of experience in cognitive therapy, preferably in a home health or community-based setting Strong assessment, documentation, and time-management skills Compassionate, patient-focused approach with excellent communication skills Familiarity with EVV systems (e.g., HHAeXchange) and MCO authorization processes preferred What We Offer Flexible scheduling - work when and where you choose Health benefits for W2 Biweekly direct deposit Consistent referrals and scheduling support A mission-driven team that values clinical excellence and independence Opportunities for growth and increased caseload as the agency expands
