Position Overview RRH is seeking a compassionate and motivated Occupational Therapist (OT) to provide per diem home health visits throughout Central Pennsylvania . This opportunity is especially well suited for an Occupational Therapist who wants to earn additional income alongside a current full-time or part-time position , gradually build a home health caseload, or explore a future transition into a more consistent or full-time role. What You'll Do As a Home Health Occupational Therapist, you will provide skilled occupational therapy services to patients in the comfort and familiarity of their homes.

Responsibilities may include:

Perform comprehensive occupational therapy evaluations and reassessments. Develop individualized plans of care based on patient needs, functional limitations, physician/provider orders, and established goals. Provide skilled occupational therapy interventions designed to improve patients' independence, safety, and functional performance. Address activities of daily living (ADLs) and instrumental activities of daily living (IADLs). Provide therapeutic activities and exercises consistent with the patient's plan of care. Evaluate home safety and recommend appropriate modifications, adaptive equipment, and assistive devices when indicated. Educate patients, family members, and caregivers regarding safety, therapeutic techniques, home programs, and strategies for maximizing independence. Monitor patient progress and modify interventions as clinically appropriate. Communicate changes in patient status to the appropriate members of the interdisciplinary care team. Coordinate care with physicians/providers, nurses, physical therapists, speech-language pathologists, social workers, home health aides, and other members of the patient's care team as appropriate. Complete clinical documentation accurately and within required timeframes. Participate in care coordination and interdisciplinary communication as required. Follow RRH policies, professional standards, and applicable state and federal home health requirements. Schedule Per Diem / Part-Time This position is designed to provide flexibility. Accept visits according to your availability. Weekday opportunities may be available. Weekend opportunities may be available based on patient needs. Begin with a small caseload if preferred. Increase your visits as your schedule and our patient census allow. Potential opportunity to transition into a more consistent or full-time caseload as RRH grows. Coverage Area Central Pennsylvania We understand that excessive travel can make home health difficult. Whenever possible, RRH works to assign patients according to your preferred coverage area and reasonable travel radius . Qualifications Current and unrestricted Occupational Therapist license in Pennsylvania . Graduate of an accredited Occupational Therapy program. Ability to provide safe, competent, patient-centered occupational therapy services. Strong clinical judgment and communication skills. Strong organizational and time-management abilities. Ability to work independently while collaborating effectively with an interdisciplinary healthcare team. Ability to complete required clinical documentation accurately and timely. Reliable transportation and ability to travel to patients' homes within your accepted service area. Current CPR certification, if required for the position. Home health experience is preferred, but qualified clinicians with relevant experience and an interest in home health are encouraged to apply.
