Job Description

Category Homehealth - OT Job Type Contract An exciting opportunity is available for a skilled Occupational Therapist with experience in home health care. This contract role is ideal for compassionate clinicians dedicated to fostering patient independence and enhancing quality of life through personalized therapy in a home-based setting.
Schedule/Hours:
Flexible scheduling to accommodate patient needs Typical daytime hours
Key Qualifications:
Valid license as an Occupational Therapist (OT) in the state of Pennsylvania Minimum of 1 year of recent home health experience strongly preferred Demonstrated ability to provide effective rehabilitation and therapy services in patient homes Proficient documentation and assessment skills Excellent communication and patient education abilities Reliable transportation and willingness to travel within the service area
Primary Responsibilities:
Conduct comprehensive patient assessments to determine individualized care plans Deliver occupational therapy interventions focused on promoting mobility, self-care, and daily living skills Track and document progress, modifying treatment plans as needed Collaborate with interdisciplinary teams including physicians, nurses, and other therapists Educate patients and their families on adaptive techniques, equipment, and safety measures Ensure compliance with federal, state, and organizational policies
