Job Description

Category Homehealth - OT Job Type Contract Deliver impactful care as a dedicated Occupational Therapist in a Home Health setting, helping individuals regain independence and thrive in their daily lives. This contract-based opportunity will allow you to support patients within the comfort of their homes, making a tangible difference through personalized, one-on-one therapy.
Key Qualifications:
Active and unrestricted Occupational Therapist (OT) license Minimum of one year of recent experience in Home Health occupational therapy Proficient with current therapy documentation systems and technology Excellent interpersonal communication and patient education skills Current CPR certification and valid driver's license Passion for creating individualized care plans and fostering patient progress
Schedule/Hours:
Flexible daytime schedule Opportunities for full-time or part-time caseload management
Responsibilities:
Conduct comprehensive assessments and develop personalized treatment plans tailored to each patient's home environment Implement evidence-based occupational therapy interventions to improve functional independence Collaborate effectively with patients, families, caregivers, and other medical professionals Document patient care, progress, and response to treatment in a timely, accurate manner Educate patients and caregivers on adaptive techniques and equipment as needed Participate in interdisciplinary case conferences to ensure high-quality care coordination
