  • PRN Skilled Home Health Emperor Health Care Services, LLC is seeking a compassionate, professional, and skilled Occupational Therapist (OT) to provide PRN skilled home health services to patients in their homes.
The OT will evaluate patients' functional abilities, develop individualized treatment plans, provide skilled interventions, and promote independence and safety with activities of daily living.
Pay Rate:
$70
  • 100 per skilled visit , based on experience, qualifications, visit complexity, and agency needs.
Responsibilities Perform comprehensive occupational therapy evaluations in the patient's home. Develop and implement individualized Occupational Therapy Plans of Care (POC) based on the patient's diagnosis, functional limitations, physician orders, and goals. Provide skilled interventions to improve activities of daily living (ADLs), instrumental activities of daily living (IADLs), strength, coordination, balance, functional mobility, cognition, and safety . Assist patients with dressing, grooming, bathing, feeding, toileting, transfers, and other daily activities to promote maximum independence. Assess the home environment and provide recommendations for safety, adaptive equipment, and appropriate modifications. Provide caregiver and family education regarding safe transfers, positioning, ADLs, home exercises, and strategies to support patient independence. Collaborate with the RN, PT, SLP, physician, case manager, and other members of the interdisciplinary team. Monitor patient progress and modify interventions as clinically appropriate. Participate in Start of Care, Recertification, Transfer, and Discharge processes when OT services are involved. Complete timely and accurate clinical documentation in the agency's electronic medical record (EHR). Document skilled need, interventions, patient response, progress toward goals, and continued need for services. Communicate significant changes in the patient's condition to the physician and agency clinical team. Assist with discharge planning and ensure patients and caregivers understand the home program and ongoing safety recommendations. Home Health Visit Expectations Provide approximately 1-hour skilled OT visits , based on patient needs and authorized services. Services are provided in the patient's home within Emperor Health Care Services, LLC's service area . Visits are scheduled on a PRN/as-needed basis according to physician orders, the Plan of Care, patient needs, and authorization. Complete required documentation following each visit. Qualifications Graduate of an accredited Occupational Therapy program. Current and unrestricted Pennsylvania Occupational Therapist license . Current CPR/BLS certification preferred/required according to agency policy. Previous home health experience preferred. Experience with Medicare home health documentation and OASIS processes preferred. Strong knowledge of anatomy, physiology, rehabilitation, ADLs, mobility, and adaptive equipment. Excellent communication, organization, and time-management skills. Ability to work independently and effectively as part of an interdisciplinary team. Reliable transportation and ability to travel to patients' homes. Compassionate and patient-centered approach to care. Preferred Experience Experience with geriatric, adult, pediatric, neurological, orthopedic, medically complex, post-hospitalization, and mobility-impaired patients is preferred. Experience with home health, acute care, inpatient rehabilitation, outpatient, skilled nursing, or pediatric settings is a plus.
