Qualifications & Experience:

Valid and active Occupational Therapist license Experience in home health occupational therapy strongly preferred Familiarity with OASIS documentation and electronic health records a plus Strong communication and interpersonal skills Ability to work independently and manage a flexible caseload CPR certification required

Responsibilities:

Conduct patient assessments and develop individualized care plans for clients receiving services at home Implement evidence-based interventions to help patients maximize functional independence in daily living activities Collaborate with physicians, nurses, and other interdisciplinary team members to coordinate care Educate patients and family members on adaptive techniques and use of assistive devices Monitor progress, revise treatment plans as necessary, and maintain thorough documentation Travel to patient homes within the assigned area
