Evaluate referred residents to determine their functional level and potential for rehabilitation. Provide direct care and develop individualized treatment plans to help residents regain independence in activities of daily living (ADLs). Establish treatment goals based on each resident's abilities, limitations, and physician recommendations. Implement therapeutic interventions to improve strength, coordination, mobility, cognition, and self-care skills. Educate residents and their families on home programs and adaptive techniques. Complete documentation including evaluations, weekly progress notes, recertifications, and discharge summaries. Collaborate with the interdisciplinary team to ensure a resident-centered approach to care. Adhere to all applicable federal and state regulations, including HIPAA and Residents' Rights. Perform additional duties as assigned by the Director of Rehabilitation.
Requirements/Qualifications:
Graduate of an accredited Occupational Therapy program. Valid Occupational Therapy license and registration for the state of practice. Experience in long-term care or geriatrics is preferred but not required. Strong communication and organizational skills. Compassionate, team-oriented, and committed to resident-centered care.
