Our center provides a four hour daytime healthcare program located in Half Moon Bay, CA 94019.
Job Title:
Occupational Therapist Reports to: Program Director This position requires a person who is a graduate of an accredited occupational therapy curriculum and is registered or eligible for registration by the American Occupational Therapy Association.
Duties:
Initial and continuing assessment of the participant, including the initial treatment plan, method and interval of delivery of services to meet identified needs. Re-assessment of participants as conditions change, and modification of treatment goals consistent with these changes. Deliver person-centered care, building a plan of care addressing the participants stated needs, goals, desires and in consideration of their abilities and strengths. Provide input to the multidisciplinary team for collaborative care planning and delivery of services. Complete TAR/ Individualized plan of care every 6 months, including a progress note for that time period. Complete quarterly assessment every 3 months, including a progress note for that time period. Complete and sign daily flow sheets, indicating therapy received and participants' response to therapy. Increase and maintain participant's capability for independence and aging in place. Enhancement of participant's emotional and social well-being. Development of participants in the use of therapeutic activities for improving function. Guidance of participants in the use of therapeutic activities for improving function. Suggest the necessary supplies and equipment to conduct occupational therapy. Provide input regarding OT policies and procedures. Other duties as assigned.
Qualifications:
A good work record, clean, neat, reliable and capable of performing above duties and others assigned. Good people skills, including: empathy and sensitivity to the needs of the elderly and disabled. Must also work well within a team model of care.
Requirements:
A bachelor's degree in occupational therapy from a university certified by the ACOTE (Accreditation Council for Occupational Therapy Education) or AOTA (American -Occupational Therapy Association). Certified by the National Board for Certification. Compliant with state healthcare regulatory requirements. Possess up-to-date knowledge of treatment practices and physical therapy programs. Excellent interpersonal communication skills. Ability to take care of clients with different personalities. Comfortable using a computer for various tasks. Ability to assess patient conditions and work with treatment plans. At least one year experience in related field
Pay:
$40.00 - $80.00 per hour
