Home Health Occupational Therapist

Job description: Pinnacle Home Care, LLC. is currently seeking an experienced Per diem Occupational Therapist (OT). We provide one-on-one skilled therapy to patients living in their homes. Occupational Therapists provides rehabilitation and skilled therapy in conjunction with the physician and the patient.

Responsibilities:

Evaluate patient needs, plan and implement patient programs that assist patients in reaching their optimal level of function. Understand and treat each patient's individual needs while educating them about their condition and developing therapy programs. Instruct and supervise patients and other healthcare personnel including Certified Occupational Therapy Assistants. Perform visits as directed in plan of care. Collaborates with all disciplines to plan and evaluate team goals for each patient. Accurately and timely document observations, interventions, and evaluations utilizing a touch pad tablet that is provided to you by the company.

Qualifications:

Current license to practice as an OT in RI. Valid CPR license. Valid driver's license and proof of insurance coverage. Ability and willingness to work in a team environment. Ability to travel between patient assignments within a 25-mile radius. We follow all RI DOH mandates and will only accept candidates that are vaccinated against Covid-19.
