Job Description

Make a Difference Where It Matters Most BrightStar Care is seeking a compassionate and skilled PRN Occupational Therapist (OT) to join our growing clinical team. This is an excellent opportunity for an OT looking for flexible scheduling, meaningful one-on-one patient care, and the ability to make a direct impact in the lives of patients in their own homes. Our OTs work closely with patients, families, caregivers, physicians, nurses, and other members of the interdisciplinary team to help patients achieve greater independence, safety, and quality of life. What You'll Do Provide skilled occupational therapy services in the patient's home according to physician orders and the established plan of care Complete comprehensive occupational therapy evaluations and develop individualized treatment plans Address functional limitations involving ADLs, mobility, strength, coordination, cognition, safety, and independence Assess patients for fall risks and home safety concerns Recommend and educate patients and caregivers regarding adaptive equipment and techniques Develop meaningful, patient-centered goals Monitor patient progress and modify treatment interventions as clinically appropriate Educate patients, families, and caregivers on home exercise programs, safety strategies, and adaptive techniques Communicate changes in patient condition and progress with the Director of Nursing and appropriate members of the care team Participate in interdisciplinary communication and care planning Complete accurate, timely clinical documentation Participate in discharge planning when appropriate Maintain compliance with all applicable federal, state, local, and agency requirements Qualifications Current, unrestricted South Carolina Occupational Therapist (OT) license Graduate of an accredited Occupational Therapy program Current CPR/BLS certification Valid driver's license Reliable transportation and current auto insurance Strong clinical assessment and communication skills Ability to work independently while collaborating effectively with the interdisciplinary team Home health experience preferred Previous experience with adult and/or geriatric populations preferred We're Looking for
