Job Description
CenterWell Home Health seeks an Occupational Therapist to deliver personalized, in-home therapy that promotes safety, independence, and quality of life. In this role, you will assess functional abilities, develop and implement evidence-based treatment plans, and provide hands-on interventions to support daily living and mobility. You will collaborate closely with an interdisciplinary team, document care thoroughly, and educate patients and caregivers.
