Job Description

Talent by Friendly Faces Workforce Solutions, LLC is recruiting on behalf of a home health client. PRN, contract, full-time, and part-time opportunities may be available. Occupational Therapist (OT) - Home Health We are seeking a dedicated and compassionate Occupational Therapist to join our team in the Houston area. The ideal candidate will provide high-quality care to patients experiencing physical, cognitive, developmental, or emotional challenges that impact their ability to perform daily activities. This position offers flexibility with full-time, part-time, permanent, and temporary options available.
Key Responsibilities:
Evaluate patients' functional abilities, limitations, and needs through comprehensive assessments. Develop individualized treatment plans designed to improve patients' independence and quality of life. Implement therapeutic interventions focused on activities of daily living (ADLs), instrumental activities of daily living (IADLs), cognitive skills, upper extremity function, and adaptive techniques. Monitor patient progress and modify treatment plans as needed to achieve optimal outcomes. Educate patients, families, and caregivers on home exercise programs, adaptive equipment, safety strategies, and techniques to maximize independence. Maintain accurate and timely documentation of evaluations, treatments, progress notes, and discharge summaries in compliance with regulatory requirements. Collaborate effectively with physicians, physical therapists, speech-language pathologists, nurses, and other healthcare professionals to coordinate patient care. Participate in continuing education and professional development activities to remain current with best practices in occupational therapy.
Qualifications:
Bachelor's, Master's, or Doctorate degree in Occupational Therapy from an accredited program. Current state licensure or eligibility for licensure as an Occupational Therapist in Texas (must be obtained prior to employment). Previous home health experience preferred but not required; new graduates are welcome to apply. Strong verbal and written communication skills with the ability to establish rapport with patients and families.
Skills :
Knowledge of rehabilitation principles related to activities of daily living, cognitive function, and adaptive equipment. Experience using electronic health records (EHR) systems. Ability to work independently while collaborating effectively within a multidisciplinary team. Compassionate, patient-centered approach to care with a commitment to improving patient outcomes.
