Job Summary:
The Occupational Therapist (OT) will evaluate, plan, and provide direct patient care services using standard occupational therapy practices and procedures. This role is responsible for developing individualized treatment plans, delivering skilled interventions, and documenting patient progress, including changes in condition and outcomes.
Key Responsibilities:
Evaluate patients and assess the impact of conditions on daily function, independence, and quality of life Interpret evaluations and develop individualized treatment plans using evidence-based interventions Provide skilled occupational therapy services to maximize safety and independence in ADLs and IADLs Assess patient environments and recommend home modifications and adaptive equipment as needed Educate patients, families, and caregivers on therapy strategies, equipment use, and functional techniques Ensure all treatments are delivered in accordance with established plans of care Monitor patient response and adjust treatment approaches as clinically appropriate in collaboration with the interdisciplinary team Collaborate with team members to ensure coordinated, high-quality patient care Maintain accurate and timely documentation in compliance with all company and regulatory standards Attend meetings as required by the Director of Rehab Follow all applicable rules, regulations, and facility procedures
Qualifications:
Experience in SNF/long-term care preferred Strong knowledge of occupational therapy practices and functional rehabilitation Excellent communication and collaboration skills Ability to work effectively with a variety of patient populations Join a team that values compassion, collaboration, and clinical excellence.
