Overview Join our dynamic healthcare team as an Occupational Therapist (OT) and make a meaningful difference in patients' lives by helping them regain independence and improve their quality of life. In this vital role, you will assess individual needs, develop personalized treatment plans, and implement evidence-based occupational therapy programs in our skilled nursing facilities. Your expertise will foster functional improvements and support clients' journey toward optimal well-being. Duties Conduct comprehensive occupational therapy assessments to evaluate clients' physical, cognitive, and emotional needs. Develop and implement tailored treatment plans that align with clinical best practices and client goals. Assist clients with activities of daily living (ADLs), including dressing, grooming, feeding, and mobility to promote independence. Support discharge planning by coordinating care transitions and providing home exercise programs or adaptive strategies. Collaborate with speech therapists, physical therapists and medical professionals to ensure holistic patient care. Manage electronic health records (EHR) accurately for documentation of assessments, treatment progress, and clinical notes. Provide specialized interventions for geriatric populations or those in occupational health settings. Requirements Valid occupational therapy license or registration in the relevant jurisdiction. Strong knowledge of anatomy, physiology, medical terminology, and evidence-based practice principles. Experience working skilled nursing facilities. Ability to develop effective care plans and treatment strategies tailored to diverse client needs. Excellent communication skills for client engagement and interdisciplinary collaboration. Familiarity with medical documentation standards and electronic health records (EHR) management.
