Job Description
Job Overview We are seeking a dedicated and energetic Occupational Therapist (OT) to join our dynamic healthcare team. In this vital role, you will assess, develop, and implement personalized occupational therapy programs that enhance patients' independence and quality of life in our Skilled Nursing Facilities. You will collaborate closely with interdisciplinary teams to deliver evidence-based care tailored to each individual's needs. Your expertise will support patients of geriatrics, including those with neurodevelopmental disorders, disabilities, or recovering from injury or illness. Responsibilities Conduct comprehensive occupational therapy assessments to evaluate clients' physical, cognitive, sensory, and functional abilities. Develop and implement individualized treatment plans focused on improving daily living activities (ADLs), motor skills, sensory integration, and overall functional independence. Assist clients with activities of daily living such as dressing, grooming, feeding, and toileting to promote self-sufficiency. Coordinate discharge planning and collaborate with healthcare teams to ensure seamless transitions across care settings. Document clinical findings accurately using electronic health records (EHR), ensuring compliance with medical documentation standards and medical terminology. Engage in case management activities including progress monitoring, adjusting treatment plans as needed, and communicating effectively with clients and their families or caregivers. Requirements Valid licensure or certification as an Occupational Therapist (OT) in the state of practice. Proven experience working with individuals with disabilities, neurodevelopmental disorders in skilled nursing facilities. Strong knowledge of anatomy, physiology, medical terminology, and evidence-based occupational therapy practices. Experience conducting occupational therapy assessments and developing effective treatment plans tailored to geriatric populations. Familiarity with implementing occupational therapy programs across various environments including inpatient acute care units, skilled nursing facilities, outpatient clinics or home health services. Ability to communicate clearly with adult geriatric clients and collaborate effectively within interprofessional teams.
