Job Description
As the PRN Occupational Therapist in Home Health, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. UnitedHealth Group Key Responsibilities Clinical Assessment & Treatment Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care. Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests. Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team. Designs and implements individualized OT treatment programs targeting ADLs, IADLs, UE strengthening, fine motor skills, cognitive function, and energy conservation. Recommends and trains patients and caregivers on adaptive equipment, home modifications, and compensatory strategies to maximize independence and safety. Fall Prevention & Home Safety Conducts home safety evaluations and recommends environmental modifications to reduce fall risk. Collaborates with nursing and PT to implement interdisciplinary home safety and fall prevention care plans for high-risk post-discharge patients. Documentation & Compliance Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition. Completes all required OT documentation, progress notes, and therapy plans of care accurately and within agency-required timeframes. Ensures documentation meets Medicare home health coverage criteria and supports medical necessity. Supervision & Collaboration Supervises occupational therapy assistants and home health aides. Participates in training, meetings, and case reviews. Refers patients to additional services as needed and coordinates discharge planning. Jooble Participates in interdisciplinary care conferences with nursing, PT, SLP, and social work as needed. Patient & Caregiver Education Provides patient care, monitors daily activities, supports routine living tasks, and helps with community reintegration and discharge planning. Required Qualifications Hold a degree from an occupational therapy curriculum accredited by the American Occupational Therapy Association (AOTA); registered with the National Board for Certification in Occupational Therapy (NBCOT). Jooble Current, unencumbered OT license Minimum 1 year of clinical OT experience; home health experience strongly preferred. Current BLS/CPR certification (AHA or ARC accredited). Valid Virginia driver's license, reliable personal vehicle, and active auto liability insurance. Ability to travel within a 15-mile service radius of Fairfax, VA. Preferred Prior home health experience with Medicare plan of care and therapy documentation. Familiarity with

Conditions of Participation, and HHPPS. Experience with fall risk assessments (TUG, SPPB, MFES) and home safety evaluations. Proficiency with electronic medical records (EMR) and point-of-care documentation. Skills & Competencies In-depth knowledge of human anatomy, physiology, and occupational therapy principles, as well as proficiency in assessment and treatment planning tools. Strong communication and interpersonal skills, empathy, and ability to adapt to diverse client needs and environments. Exposure to variable home conditions; possible contact with blood, body fluids, or infectious materials. Reliable transportation required for field-based visits. Work Environment Field-based PRN position. Hours are as-needed with no guaranteed minimum. OTs are expected to respond to scheduling requests promptly and maintain professional reliability.
