Whether your background is in outpatient therapy, acute care, inpatient rehabilitation, skilled nursing, or home health, PACE offers a unique opportunity to practice occupational therapy through a holistic, participant-centered model of care. What You'll Do As a PRN Occupational Therapist, you'll work closely with physicians, nurses, physical therapists, social workers, dietitians, home care professionals, and other members of the Interdisciplinary Team to support participants' functional independence and individualized care goals.
Responsibilities include:
Perform occupational therapy evaluations and reassessments Develop individualized treatment plans focused on activities of daily living (ADLs), instrumental activities of daily living (IADLs), upper extremity function, cognition, safety, and functional independence Provide therapeutic interventions to improve or maintain participant function Assess home environments and recommend adaptive equipment, assistive devices, and environmental modifications Educate participants, caregivers, and team members regarding therapeutic techniques and safety strategies Participate in interdisciplinary care planning and case conferences, as appropriate Maintain timely and accurate clinical documentation Collaborate with the Interdisciplinary Team to provide participant-centered care
Qualifications Required:
Graduate of an accredited Occupational Therapy program Current Virginia Occupational Therapist license or eligibility for licensure Current BLS certification Valid driver's license and reliable transportation At least one (1) year of experience working with a frail or elderly population, or completion of a PACE organization-approved training program that addresses the needs of the frail elderly prior to assuming independent responsibilities. Strong communication and interdisciplinary collaboration skills
Preferred:
Experience in geriatrics, outpatient rehabilitation, acute care, inpatient rehabilitation, skilled nursing, home health, or PACE Experience completing home safety evaluations and recommending adaptive equipment Experience working with medically complex older adults Passion for participant-centered care Share job details to
