The ideal candidate will work with individuals with disabilities, providing therapeutic interventions to enhance their ability to perform daily activities. This role involves collaborating with patients, families, and other healthcare professionals to develop and implement personalized treatment plans that promote independence and improve quality of life. Initially this is a Part Time/ PRN position offering 10-15 visits per week, with the possibility of transitioning to Full Time in the future. Duties Conduct assessments to evaluate patients' needs and establish appropriate therapy goals. Develop individualized treatment plans based on patient evaluations and progress. Implement therapeutic interventions in various settings, including home health, clinics, and acute care facilities. Educate patients and their families on treatment strategies and techniques to facilitate recovery. Monitor patient progress and adjust treatment plans as necessary to achieve desired outcomes. Collaborate with interdisciplinary teams, including speech therapists and case managers, to provide comprehensive care. Maintain accurate documentation of patient assessments, treatment plans, and progress notes in accordance with medical standards. Stay updated on best practices in occupational therapy, including knowledge of anatomy and physiology as well as medical terminology. Qualifications Degree in Occupational Therapy from an accredited program. Valid state licensure or certification as required by law. Experience working with individuals with disabilities is preferred. Knowledge of home health practices, acute care environments, early intervention strategies, and case management processes is a plus. Strong understanding of anatomy and physiology to inform treatment approaches. Excellent communication skills for effective collaboration with patients, families, and healthcare teams.
