Job Description
Responsibilities:
Assess patients' needs and develop treatment plans to help them reach their goals. Implement interventions to help patients improve their physical, cognitive, and emotional functioning. Educate patients and their families about their conditions and the treatment plan. Monitor patient progress and adjust treatment plans as needed. Collaborate with other healthcare professionals to coordinate care. Provide guidance on how to use adaptive equipment and other assistive devices. Assist patients in developing the skills necessary for daily living activities.Job Type:
