Long-term care / skilled nursing Shared two-location assignment: 3 days at one site, 2 days at the other Responsibilities Conduct occupational therapy evaluations for adult residents and develop individualized treatment plans focused on functional mobility, activities of daily living (ADLs), and participation in meaningful tasks. Implement and supervise therapy interventions to improve strength, range of motion, balance, coordination, and independence in daily activities. Communicate findings and progress with the interdisciplinary team; participate in discharge planning and caregiver education. Maintain accurate documentation of evaluations, treatment plans, progress notes, and outcomes in accordance with regulatory and facility standards. Manage a shared schedule across two local facilities (3 days at one site, 2 days at the other) to ensure continuity of care and appropriate patient coverage. Adhere to facility policies and safety/infection control practices while delivering high-quality patient care.
